General Patient Information

This leaflet is given out to every new patient, providing a general overview of the practice. It will be updated from time to time. Should you wish additional copies then please ask your podiatrist.

New Patients

As a new patient to the the practice your first appointment will be via a virtual consultation in the form of either a phone or video consultation.  Please download the two forms below. Once downloaded print and complete by hand, before returning either a scanned copy or a photo from your phone via email to dorsetpodiatry@hotmail.com before your session. If you aren’t able to do this then that is no problem, the podiatrist can take you through them during the appointment.

Treatment of Children

 

By law, those under the age of 16 require someone with the right of parental consent to be present during their treatments. This means that a parent (or someone granted the right of parental consent by the courts) must be present during treatment.
If this is not possible then we have created a special form that will allow another adult to bring the patient so long as the parent will be available by phone during treatment. We appreciate that this will be an inconvenience to some, but we have a professional responsibility to ensure that we carry out treatments in accordance with the law so we thank you for your understanding.
Failure to meet these conditions will regrettably result in the child’s treatment not going ahead.

For ease all of our children’s forms are available for download here. We would strongly recommend that a parent accompanies the child to the first treatment but if this is not possible, please download and complete the 2 new patient forms as well as the Absence of Parental Consent form and send those along to the treatment.
